Local Coordinating Council

The Local Coordinating Council (LCC) includes local representatives from each of the local child-serving county agencies, the family and a parent from the community where the council monitors a child’s placement to ensure child’s needs and goals are being met. The LCC’s main goal is to assure a child’s placement is the least restrictive environment and an appropriate placement for each individual child, whether it is a residential placement or a more suited community service program.

This survey is being used to gather your opinions about the school-based health center (SBHC) in your child’s school.  The information you provide will be used to improve services offered at the SBHC.  Your answers will be kept confidential.  If you have more than one child who attends this school, we ask that you complete and return a survey for each child.  You are not required to answer these questions, and if you choose not to do so, it will not affect your ability or your child’s ability to use health services at the SBHC.  Thank you for sharing your thoughts with us!
